In this video, you'll learn about the differences in left-sided vs right-sided heart failure, the symptoms, pathophysiology, causes, and more. In left-sided heart failure, the weak heart allows blood to flow back into the lungs, which causes pulmonary congestion. Systolic heart failure features a REDUCED ejection fraction, whereas diastolic heart failure has a PRESERVED ejection fraction. Left-sided heart failure can lead to symptoms such as shortness of breath, crackles, orthopnea, difficulty breathing, and more. Right-sided heart failure is usually caused by left-sided heart failure due to the increased fluid pressure backing up from the left side to the right side. Stroke nursing NCLEX review (CVA) cerebrovascular accident lecture on ischemic and hemorrhagic strokes along with nursing care, tPA, symptoms, treatments, and pathophysiology. A stroke occurs when blood rich in oxygen can NOT reach brain cells (due to either a blockage or bleeding) and the cells begin to die. Types of stroke: Ischemic (most common) is due to a blood clot within the blood vessel or stenosis of an artery feeding the brain tissue. This limits the blood that can reach the brain cells. This can happen due to: -Embolism: where a clot has left a part of the body (example the clot has left the heart due to a heart valve problem or atrial fibrillation) and has travelled to the brain, which stops blood flow. -Thrombosis: a clot forms within the artery wall within the neck or brain. This is seen in patients with hyperlipidemia and atherosclerosis Hemorrhagic: there is bleeding in the brain due to a break in a blood vessel. There will be limited blood flow to the brain cells and this can cause excessive swelling in the brain from the leakage of blood in the brain tissue. A ruptured brain aneurysm or uncontrolled hypertension can cause this. In addition, a patient can experience a TIA (transient ischemic attack) also called a mini-stroke. This is where signs and symptoms of a stroke last only a few minutes to hours and resolve. It is a warning signs an impending stroke may occur. Stroke symptoms include: facial drooping, arm numbness or weakness, slurred speech or difficulty talking etc. Treatment for stroke (specifically an ischemic type) is tPA (tissue plasminogen activator). This medication works to dissolve the clot that has caused th decreased blood flow. However, tPA must be administered within 3 hours of the onset of stroke symptoms. Nursing care for stroke includes: monitoring vitals (especially blood pressure, respiratory status), watching and educating the patient with neglect syndrome (this most likely occurs with patients who've had a right brain injury), diet and swallowing techniques, hemianopsia, and how to communicate with a patient with aphasia (watch the video for more in depth nursing interventions for stroke). Clinical presentation of hypovolemic shock include; tachycardia, hypotension, cold extremities, clammy skin, reduced urine output, and altered mental status. Hypovolemic shock can be managed through correction of the fluid deficit and treatment of the cause. Fluid therapy can be via crystalloid, colloid, blood, or blood products. Nursing interventions entail monitoring the vital signs including circulation, and oxygen saturation (sPO2), nursing the patient in modified Trendelenburg position, fluid therapy via large gauge intravenous access bilaterally etc. #shock #typesofshock #nursing #nclex

Puncția venoasă – Scop Explorator: recoltarea sângelui pentru examene de laborator; Terapeutic: administrarea medicamentelor sub formă de injecție sau perfuzie intravenoasă, transfuzii, sângerare în hipertensiune sau edem pulmonar acut. Locuri de elecție pentru puncția venoasă Venele de la plica cotului (bazilică şi cefalică). Venele antebrațului. Venele de pe fața dorsală a mâinii. Venele subclaviculare. Venele femurale. Venele maleolare interne. Venele jugulare şi epicraniene la copil şi sugar. Materiale necesare Perna pentru sprijinirea brațului pacientului Tampoane sterile Alcool sanitar Garou Mănuși sterile Seringi în funcție de scop Medicamente (dacă se administrează) Tăvița renală Bandaj pentru locul puncției Pregătirea pacientului Se cere consimțământul pacientului; I se explica tehnica și necesitatea acesteia; Se asigură confort fizic și psihic; Se așază pacientul într-o poziție comodă, luând în calcul și posibilitatea lipotimiei. Tehnica puncției venoase Se așază brațul pe pernă, în extensie; Se examinează calitatea și starea venelor; Se evită locurile cu răni, cicatrici, hematoame sau edeme; Se palpează vena și se alege locul puncției; Asistenta își pune mânuși de protecție; Se aplică garoul la 7-8 cm deasupra locului de puncție; Se dezinfectează locul ales cu mișcări circulare de la locul puncției, în afară; Se recomanda ca pacientul sa strângă pumnul pentru ca venele să devina turgescente; Se fixează vena cu policele mâinii stângi la 4-5 cm sub locul puncției, exercitând o ușoară compresiune în jos asupra țesuturilor vecine; Se ia seringa cu mâna dreaptă între police și restul degetelor; Se pătrunde în vena cu acul menținând un unghi de aproximativ 30 de grade , învingând-se o rezistență elastică a pielii, până când acul înaintează în gol cca 1-2 mm, cu bizoul în sus și gradațiile seringii în sus; Se verifică poziția acului în venă prin aspirare în seringă; Se continua tehnica în funcție de scopul urmărit; Se cere bolnavului să deschidă pumnul și se îndepărtează garoul; Se retrage acul brusc și se aplică un tampon steril la locul puncției; Se menține tamponul cca 1-3 minute, fără a se îndoi brațul. Accidente/ incidente hematom, prin infiltrarea cu sânge în țesutul perivenos. Se retrage acul și se comprimă locul 1-3 minute perforarea venei. Se retrage acul. amețeală, paloare, lipotimie – se întrerupe tehnica și se acordă ajutor sau se cheamă medicul. Since I began watching medical dramas for the reacts series, I've found more and more that nurses get overlooked within media. Being a family medicine physician I can very much empathize with the situation. The dedication and contribution to healthcare from both fields is incredibly understated. Studying to become a nurse is a noble undertaking and it is a career with so many positives that I decided to make a dedicated video to all the amazing nurses out there. Instead of just me talking about nurses I decided to vlog with some of my staff I've been working with for the last few years to give you some first-hand insights into the career. Obviously, some of the nurses are a bit camera shy, which is understandable but I think they did a great job highlighting whats its like to be a nurse and in general about being a nurse. I know you guys have wanted a hospital vlog for some time now but as you can imagine vlogging in a hospital can be quite difficult with patient privacy and all. But for nurses week 2018 I had to make an exception and interview my colleagues. Also at the end of the video, you'll see many of the doctors I've worked or am still working with. The head-to-toe assessment in nursing is an important physical health assessment that you'll be performing as a nursing student and nurse. Head-to-toe assessments allow nurses to assess the health status of patients by following a checklist of criteria. On the job, your head-to-toe nursing assessment will be performed much faster, and it may be different or more specialized to accommodate the patients' needs within your nursing specialty. This assessment represents a general assessment checklist (or cheat sheet) that you might encounter in nursing school.
